My Health Teams and Team Based Care is an approach to care that brings together different health care providers to provide the best care for you, close to home. While all My Health Teams have shared characteristics and approaches to care, every team is different. My Health Teams are developed locally and based on your needs and your community’s needs.
Southern Health-Santé Sud has four My Health Teams located in:

- Morden/Winkler
- Portage/Gladstone
- Mon équipe santé – linking Francophone communities
