Occurrence Reports for COVID-19 Positive cases
We have been directed by Manitoba Health, Seniors and Active Living (MHSAL) to report the following matters as Critical Occurrences (CO):
- Residents from PCH’s who have tested positive for COVID-19,
- Residents from PCHs and patients from acute care who have passed away as a result of COVID-19 and
- Staff who have tested positive for COVID-19.
When completing the Occurrence report for reporting residents from PCHs who have tested positive for COVID-19 and resident/patient deaths from COVID-19 indicate the date the staff was informed that the residents/patient was positive for COVID-19 or the date the resident/patient passed away.
On the second page under Miscellaneous check off “Other” and write “Significant public health event that may affect the delivery of health care services and programs/may create media attention”.
The occurrence report is to be scanned to the Regional Coordinators- Patient Safety for reporting the Significant Public Health Event to MHSAL.
For reporting Staff who have tested positive for COVID-19
For the date/time of the occurrence: indicate the date/time the staff member was informed of the positive COVID-19 test result
On the second page under Miscellaneous check off “Other” and indicate “Significant public health event that may affect the delivery of health care services and programs, likely to create media attention”.
The CO is not the staff injury but the significant public health event.
The occurrence report is to be scanned to Occupational Health and Safety as noted on the occurrence report along with the Regional Coordinators- Patient Safety for reporting the Significant Public Health Event to MHSAL.
Directors of Health Services and Program Directors (or delegates) are to review this process and share with all applicable staff.
Alternative Isolation Accommodations (AIA)
Staff that may make referrals are to be aware of the following:
- There are two AIAs:
- One offers health care workers and other Manitobans who have tested positive for COVID-19 or are close contact, a place to self-isolate if they do not have access to a private bedroom and bathroom in their home or if they live with someone who is at greater risk from COVID-19 (e.g. immune compromised). This first AIA has health care staff days and evenings from 8:00 a.m. to 8:00 p.m.
- The second location offers individuals experiencing homelessness a space to self-isolate if they tested positive for COVID-19 or if they are suspected. It has 24 hour supports including harm reduction supplies and resources.
- Referrals of people with children that meet the criteria will be accepted. These families will be located at the first AIA, with 12-hour health care staff.
- You do not need to determine which AIA the referral is for, call or forward the referral to the AIA Intake Coordinator, and the Intake Coordinator will determine which site
- If you need to make a referral you can do so by completing the AIA Referral Form found here - and email or call the Intake Coordinator at 204-795-3093.
- The AIA will arrange transportation to and from the AIA if required.
- Referrals are accepted 7 days a week from 8:30 a.m. to 4:30 p.m.
- If you are making referral later in the afternoon and the individual will not be able to get to the AIA by 4:30 then the referral may not be accepted and arrangements can be made for the individual to go the following day.
- If the person being referred is quite ill, a health assessment may be required before they are sent to the AIA to ensure they are able to stay at the AIA and do not need to be at a hospital.
